Central Minnesota Palliative Care

Improving Quality of Life for Patients With Chronic Illnesses

If you or a loved one has a chronic illness, it’s exhausting but help is available.

Palliative care is an additional layer of support for patients diagnosed with serious illness who seek relief from their symptoms and stress. Palliative care can be provided along with curative treatment. Our team of providers, nurses and chaplains work together with patients, their families and patients’ providers to improve the quality of life throughout their journey.

What You Can Expect From Palliative Care:

  • Relief from symptoms, including pain, shortness or breath, fatigue, constipation, nausea, loss of appetite, numbness and problems with sleep.
  • In-depth communication with you and your family about your goals, concerns and your treatment options.
  • Coordination of care among your health care team.
  • Improved ability to carry on with your daily life.
  • Emotional, spiritual and practical support for you and your family.

Treatment from a Team of Experts

The Palliative Care team will meet with you and your family to understand your needs. We will make a plan that respects your values and personal choices. Your primary care doctor will remain in charge of your care. Your team could include:

  • A doctor or advanced practice nurse will review and treat your symptoms, give treatment choices and set goals for care.
  • Nurses manage the efforts of the Palliative Care team. They track your symptoms and response to treatment. They are directly available to give extra support to you and your family.
  • Social workers help you and your family with discharge planning, community resources and relationship issues.
  • Chaplains will listen, pray and offer coping strategies and counseling. They give support based on your faith, values and spiritual needs.
