My Heart Program

My Heart Program can:

  • Help you understand heart failure
  • Support you in eating healthier, exercising more and quitting smoking
  • Improve your daily life by finding the right medicines and treatments
  • Reduce emergency room visits and hospital stays with regular check-ins

You have support through in-person and video visits, phone calls and a help line. You might also get a device to monitor your health at home, which can help prevent hospital visits.

More than 2,500 Minnesotans receive care from us each year at sites conveniently located in Alexandria, Melrose, Monticello and St. Cloud, along with telehealth services in Willmar. A dedicated heart failure team provides personal treatment, taking the time to get to know you. Our nurses are certified in heart failure, meaning they have in-depth knowledge of the best ways to help you.
