Income-Based Financial Assistance
CentraCare is committed to providing high-quality health care to the community regardless of ability to pay.
As part of our commitment to serving the health needs of people who are uninsured or underinsured, CentraCare offers discounts to those whose incomes are less than 250% of the Federal Poverty Guidelines.
All income-based program applicants will also be screened for the Hospital Department Discount Program.

Applying for Financial Assistance
To apply for financial assistance, patients must submit a complete application (including supporting documents):
- via MyChart,
- via email (to financialassistance@centracare.com),
- in person (at any CentraCare entity), or
- by mail (to CentraCare, Attn: Patient Financial Services, 1406 6th Ave N, St. Cloud, MN 56303).

Hospital Department Discount Program
All Income-Based Financial Assistance Program applicants will automatically be screened for the Hospital Department Discount Program. This discount only applies to services received in a CentraCare hospital department.
You may be eligible for this discount if you meet the following criteria:
- You do not have insurance or if you have received services in a hospital department that your insurance determines are not covered.
- Your yearly household income is below $125,000 and you are not already covered by our financial assistance program (detailed above).
