EpicCare Link
EpicCare Link is a web-based, read-only version of CentraCare's electronic health record, Epic. EpicCare Link partner organizations are able to log in and view a patients' charts, including medication list, progress notes, radiology reports and EKG interpretations and discharge summaries.
EpicCare Link is offered to skilled nursing facilities, assisted living facilities, home health agencies, clinics or medical offices that refer patients to CentraCare. EpicCare Link is designed to provide access to patient medical records to assist in the transition of care to a qualified medical facility.
For Existing EpicCare Link Users
Are you an end user wanting EpicCare Link access or requesting your login to be reactivated?
Please work with your EpicCare Link site liaison to submit the enrollment form to initiate the setup process.
Are you an active CentraCare EpicCare Link end user and are unable to login (forgot username or password)?

You will be provided with the EpicCare Link Guidelines, EpicCare Link User Agreement and a series of questions that will help us review your request, which may take two to six weeks depending on the type.
Following CentraCare's review, you will be notified via email by a member of the CentraCare EpicCare Link team regarding the next steps.
Whose information will be available?
With CentraCare's EpicCare Link, partner organizations are able to view select patients’ charts from all sites utilizing Epic, our electronic health record. This includes patients from our regional facilities:
Long Prairie, Melrose, Sauk Centre, Monticello, Paynesville, Rice Memorial in Willmar, Redwood Falls, Swift County Benson, Alliance Imaging and associated clinics. It also includes CentraCare's Epic affiliate sites: Alomere Health, Tri-County Hospital (Astera Health), Madison Hospital, Stevens Community Medical Center, St. Gabriel’s Hospital, Johnson Memorial, Appleton Area Health Services, CCM Health, Meeker Memorial, Williams Integracare and Allergy, Asthma and Pulmonary Associates, Simplicity Health.
EpicCare Link helps CentraCare and our affiliates to extend patient information to other entities. This sharing of information can help improve the continuity of care for patients in our community and eliminate the need for faxing.
What information is not available in EpicCare Link?
At this time, radiology images, EKG tracings and some behavioral health documentation is not viewable in CentraCare's EpicCare Link.
How much does EpicCare Link cost?
This is a free program. No cost. No installment fee, no training fee, no maintenance fee.
